How to File a Request
The Illinois Freedom of Information Act​, (FOIA) requires that all public bodies make public records available to the public, subject to certain exemptions. It is the policy of the Streamwood Park District to fill all requests for public records in as timely a fashion as possible. By law, the maximum time to produce the information for routine requests is five business days; commercial requests are 21 business days. In rare cases, you will be notified, in writing, if an extension is needed, as allowed by the Act.
You must disclose whether your request is for a commercial purpose. It is a violation of the Freedom of Information Act for a person to knowingly obtain a public record for a commercial purpose without disclosing that it is for a commercial purpose, if requested to do so by the public body. 5 ILCS 140.3.1(c)).
In compliance with the Freedom of Information Act, the Streamwood Park District’s Freedom of Information Officers are Sherri Harry and Elizabeth Sampson. All FOIA requests must be submitted in writing. To make the process easier and faster, we encourage you to use our optional FOIA Request form on this page. Use of this specific form is not mandatory, although all required information below must be included in any written request. While the online form is the preferred method, we accept all written requests regardless of format to submit by mail, fax, or email
